Apolipoprotein A-1 is an apolipoprotein — a protein that binds lipids to form lipoprotein particles. It is produced mainly by the liver and intestine and is encoded by the APOA1 gene. Biochemically, ApoA-1 is the dominant structural and functional protein of HDL and interacts with enzymes such as LCAT to support lipid processing. Functionally, ApoA-1 plays a central role in reverse cholesterol transport, helping remove excess cholesterol from cells and macrophages and delivering it for excretion. Causes that can be associated with lower values include certain genetic variants, insulin resistance, chronic inflammation, smoking, or altered lipid metabolism. Higher ApoA-1 levels usually indicate a greater HDL particle presence or specific genetic influences and can be seen with favorable lipid patterns, but elevated levels do not automatically equate to effective HDL function. Acute-phase changes, laboratory variability, or uncommon genetic forms of HDL can also affect levels.
