Overview and History
CYP2C19*2 corresponds to the single nucleotide polymorphism rs4244285 in the CYP2C19 gene, part of the cytochrome P450 family of phase I drug‑metabolizing enzymes. The *2 allele creates an abnormal splice site that produces little or no functional CYP2C19 protein, which alters oxidative metabolism of many small molecules.
Researchers recognized CYP2C19*2 as a major cause of the ‘‘poor metabolizer’’ pattern when pharmacogenomics emerged as a clinical field. The allele frequency varies by ancestry, being more common in some East Asian populations and less common in many European groups, so population context matters when interpreting results.
Interpreting Highs and Lows
For a genetic marker like CYP2C19*2, ‘‘levels’’ refer to whether the variant is present in zero, one or two copies. No copies (variant absent) is associated with normal CYP2C19 activity; one copy often yields intermediate activity; two copies are associated with markedly reduced or absent activity.
