Overview and History
Non-HDL-C is a calculated lipid value: total cholesterol minus HDL cholesterol. It represents cholesterol carried by LDL, VLDL, IDL, lipoprotein remnants and Lp(a) — collectively the atherogenic lipoprotein pool.
Clinically, Non-HDL-C has been recognized as a robust marker of atherogenic cholesterol burden across a range of metabolic states, especially when triglycerides are elevated. Biochemically, it classifies as a lipoprotein-derived cholesterol metric and serves as a practical summary of particle-associated cholesterol transport in the circulation.
Interpreting Highs and Lows
Elevated Non-HDL-C suggests a higher burden of cholesterol carried by atherogenic particles and is commonly seen alongside patterns such as high triglycerides, altered lipoprotein metabolism, or inherited lipid traits. Low Non-HDL-C generally indicates a lower atherogenic particle cholesterol burden and may occur with low overall cholesterol production, certain metabolic states, or the effects of lipid-lowering interventions. Interpretation is most meaningful when combined with other Cardio Zoomer markers (ApoB, triglycerides, lipoprotein(a), genetic variants and metabolic/inflammatory measures) to define underlying patterns and drivers.
