Overview and History
Oxidized LDL is a biochemical form of low-density lipoprotein whose lipid and protein components have been chemically modified by reactive oxygen species. Biochemically, oxLDL contains oxidized phospholipids and altered apolipoprotein B structures that are detectable by specific immunoassays.
Research linking oxLDL to arterial biology emerged from studies of oxidative stress and atherosclerosis; oxLDL is studied as a marker of lipid oxidation and related vascular processes. Functionally, oxidized LDL interacts with endothelial cells and immune cells, promoting uptake by macrophages and triggering signaling pathways tied to inflammation and local arterial changes.
Interpreting Highs and Lows
Elevated oxLDL levels are commonly associated with greater oxidative stress, increased inflammatory signaling, and a higher proportion of modified, potentially more reactive LDL particles. Associations are seen with metabolic strain, higher LDL particle burden, smoking, and factors that raise oxidative load. Low oxLDL levels may reflect lower oxidative modification relative to LDL concentration, effective antioxidant defenses, or simply lower LDL particle numbers. Interpretation should be done alongside lipid particle measures, inflammatory and redox markers, and clinical context to clarify what the value means for an individual.
