Cholesterol belongs to the sterol class of lipids and is produced mainly by the liver while also coming from dietary sources. Total cholesterol quantifies the sum of cholesterol contained in these particles and is reported in standard lab units. TC measurement has been used in clinical practice for decades. Large epidemiologic studies identified consistent associations between higher total cholesterol levels and long-term cardiovascular outcomes, which led to routine lipid screening. Today TC remains a fundamental lab value and is interpreted together with particle-specific measures (ApoB, LDL particle number, Lp(a)), triglycerides, and newer markers to clarify the underlying lipid biology.

Elevated TC suggests a higher total burden of cholesterol carried by circulating lipoproteins and may be associated with greater numbers or cholesterol content of atherogenic particles, increased synthesis or absorption, metabolic patterns (such as insulin resistance), inflammatory states, or inherited factors. However, the clinical meaning of a high TC depends on the breakdown of that total into LDL, HDL, triglycerides, ApoB, Lp(a), and other panel results. Low TC can reflect reduced hepatic production, malabsorption, certain metabolic or nutritional states, or effects of medications and recent illness; in some cases it reflects a favorable lipid profile when HDL is high and atherogenic markers are low.
