Overview and History

Citric acid, chemically citrate in its ionized form, is the first product of the mitochondrial tricarboxylic acid (TCA or Krebs) cycle, formed when acetyl‑CoA combines with oxaloacetate via citrate synthase. Citrate operates as a central metabolic node: it donates carbon for fatty acid and sterol synthesis when exported to the cytosol, allosterically regulates glycolysis, and can chelate metal ions. The TCA cycle was characterized by Hans Krebs in the 1930s, highlighting citrate’s role as a foundational metabolite in cellular respiration. Clinically and physiologically, citrate levels—especially in urine—reflect both mitochondrial production and the body’s handling of acid–base load and renal citrate excretion, making it a useful biochemical marker of metabolic state.

Interpreting Highs and Lows

Higher-than-typical citrate levels in urine or excreted metabolites can reflect increased production, reduced downstream utilization, changes in cellular export to support biosynthesis, or alterations in renal excretion. Elevated values may also follow recent dietary citrate intake. Lower-than-typical citrate can be associated with reduced Krebs cycle flux, greater mitochondrial substrate demand, diversion of citrate into biosynthetic pathways, or changes in acid–base balance that increase renal citrate reabsorption.
