Overview and History
Anti-TPO (anti–thyroid peroxidase) belongs to the class of autoantibodies—immunoglobulins that bind a self protein. Specifically, these antibodies recognize thyroid peroxidase (TPO), an enzyme located in thyroid tissue that plays a central biochemical role in making thyroid hormones.
Historically identified as “microsomal” antibodies in mid-20th-century research, anti-TPO became a key laboratory marker for immune-mediated activity involving the thyroid. As a lab measure it does not assess hormone levels directly but indicates whether the immune system is targeting a protein essential to thyroid hormone synthesis.
Interpreting Highs and Lows
Elevated anti-TPO levels suggest increased immune recognition of thyroid peroxidase. In practice this often occurs alongside changes in thyroid hormone measurements or symptoms related to altered thyroid function, but elevated antibodies alone reflect immune activity rather than a measured hormone deficit.
Low or undetectable anti-TPO levels indicate a lack of measurable autoantibody targeting TPO. Normal or low anti-TPO does not guarantee normal thyroid hormone status; interpretation is most informative when integrated with thyroid hormones, TSH, and the broader Foundation Zoomer panel.
