Overview and History

Apolipoprotein B (Apo B) is a large structural protein present mainly as ApoB-100 (made by the liver) and ApoB-48 (made by the intestine). It is the primary apolipoprotein on very-low-density, intermediate-density, and low-density lipoprotein particles and forms the backbone of those particles. Recognized clinically for its relationship to particle number, Apo B is useful because each atherogenic lipoprotein particle carries one Apo B molecule. Biologically, Apo B provides structural stability to lipoproteins and plays a role in interactions with cell surface receptors involved in lipid transport and clearance.

Interpreting Highs and Lows

Higher Apo B levels generally reflect a larger number of atherogenic lipoprotein particles in circulation. This pattern can be associated with elevated triglycerides, impaired particle clearance, certain inherited lipid traits, or metabolic states such as insulin resistance, indicating increased particle-driven lipid burden even when cholesterol concentrations are not markedly high.
