Overview and History
Apo B:Apo A-1 is a biochemical ratio of two apolipoproteins: ApoB, the primary structural protein of LDL, VLDL and other atherogenic particles, and ApoA-1, the main protein of HDL. The ratio was developed to estimate the relative burden of particles that deliver cholesterol versus those that participate in its removal, offering a particle-focused view of lipid biology.
Biologically, each atherogenic particle typically carries one ApoB molecule, so ApoB correlates with particle number; ApoA-1 supports reverse cholesterol transport and has antioxidant and anti-inflammatory roles. Together the proteins summarize opposing lipoprotein functions, making their ratio a compact marker of lipoprotein balance and particle quality.
Interpreting Highs and Lows
Elevated Apo B:Apo A-1 reflects a higher proportion of ApoB-bearing particles relative to ApoA-1. This pattern is associated with greater numbers of atherogenic lipoprotein particles and often aligns with higher triglycerides, markers of insulin resistance, or inflammatory signals.
Low Apo B:Apo A-1 indicates a lower proportion of ApoB relative to ApoA-1 and is generally associated with a more favorable lipoprotein balance. Both high and low values should be interpreted within the full Foundation Zoomer panel—lipid subfractions, hs-CRP, Lp(a), glucose/insulin markers and clinical context—to clarify underlying patterns.
