Overview and History

Cortisol is a glucocorticoid steroid synthesized from cholesterol in the adrenal cortex. It is released into the bloodstream under control of the hypothalamic–pituitary–adrenal (HPA) axis — a signaling pathway that uses CRH and ACTH to adjust adrenal output based on circadian cues and stress-related signals. Historically characterized in the early 20th century as clinicians isolated adrenal steroids, cortisol’s core biological actions include shifting fuel use (glucose, protein, and fat), modulating inflammatory and immune activity, and supporting vigilance and blood pressure regulation. Its secretion follows a strong daily rhythm, typically peaking in the morning and falling through the evening.

Interpreting Highs and Lows

Elevated cortisol levels may be associated with prolonged physiological or psychological stress, disrupted sleep–wake cycles, higher circulating glucose levels, altered fat distribution, or shifts in immune markers. Low cortisol readings may be associated with persistent fatigue, reduced ability to mount a normal stress response, or symptoms tied to low circulating steroid activity.
