Creatinine is a non-protein nitrogen compound formed from the non-enzymatic conversion of creatine and creatine phosphate in muscle. Chemically it is a cyclic amine derived from creatine; it is water‑soluble and cleared from the body primarily by glomerular filtration in the kidneys and, to a lesser extent, by tubular secretion. Clinically, serum and urine creatinine measurements have long been used as accessible markers of filtration and renal handling of waste. Because creatinine production is tied to muscle mass and turnover, its interpretation blends information about muscle biology, fluid status, and renal clearance rather than representing a single isolated process.

Interpreting Highs and Lows

Higher-than-average serum creatinine values often correlate with reduced filtration capacity, lower effective blood volume (e.g., dehydration), greater muscle breakdown, or factors that slow excretion. Certain medications and acute changes in kidney perfusion can also shift levels. Lower-than-average creatinine can be associated with lower muscle mass, reduced dietary protein intake, pregnancy, or increased clearance; very low values may reflect generalized low muscle turnover or dilutional states.
