DHEA-S is a sulfated steroid hormone—chemically a sulfate conjugate of dehydroepiandrosterone. Sulfation increases water solubility and extends circulation time, so DHEA-S is the predominant, measurable adrenal androgen circulating in blood. The majority is produced by the adrenal cortex with smaller contributions from gonadal tissues; peripheral tissues can convert it into active androgens and estrogens. Clinically, DHEA-S became useful because its sulfate form is more stable than the unconjugated parent compound, making it easier to compare across patients and over time. Levels normally peak in early adulthood and decline with age, so patterns of elevation or decline are interpreted relative to age, sex, and accompanying hormone markers.

Higher-than-expected DHEA-S levels are associated with increased adrenal androgen output or exposure to androgenic substances and can signal an androgen‑dominant hormonal pattern when seen alongside raised downstream androgens.
