Overview and History

eGFR (estimated glomerular filtration rate) is a calculated value rather than a direct laboratory analyte. Common equations use serum creatinine—often combined with age, sex, and sometimes cystatin C—to translate a blood concentration into an estimate of filtration speed. Historically, formulas such as Cockcroft–Gault, MDRD, and CKD‑EPI were developed to standardize assessment of kidney filtration across populations. Biochemically, eGFR relates to glomerular filtration, the kidney’s process of moving water and small solutes from blood into the urine, which supports waste removal, electrolyte balance, and fluid regulation.

Interpreting Highs and Lows

Lower eGFR values are associated with reduced filtration capacity or lower blood flow through the kidneys; causes can include reduced perfusion, structural changes, dehydration, or effects of certain medications. Higher or unexpectedly elevated eGFR values can reflect hyperfiltration states, physiologic changes such as pregnancy, or analytic artifacts—especially when low muscle mass lowers serum creatinine and overestimates filtration. Interpretation of eGFR is most informative when considered with creatinine, BUN, electrolytes, urine studies, medications, and the broader clinical pattern in the Foundation Zoomer.
