Overview and History

Free T3 (free triiodothyronine) is a small iodinated hormone in the thyroid hormone family. Biochemically it is the unbound, biologically available fraction of T3; most T3 in blood is protein-bound, while Free T3 is the portion that can enter cells and bind nuclear thyroid receptors. Most circulating T3 is produced outside the thyroid by enzymatic conversion of T4 to T3 via deiodinase enzymes. Free T3 modulates cellular energy use, gene expression linked to metabolism, heart rate, thermogenesis, and aspects of brain function, making it a central readout of tissue-level thyroid signaling in clinical evaluation.

Interpreting Highs and Lows

Higher Free T3 may reflect increased circulating active thyroid hormone signaling or greater conversion of T4 to T3. Elevated results can be associated with medication effects, changes in hormone production or conversion, physiologic stressors, or laboratory variation; interpretation depends on TSH, Free T4, reverse T3, antibody markers, and clinical context. Lower Free T3 may indicate reduced tissue-level thyroid signaling, decreased peripheral conversion of T4 to T3, acute illness effects, medication interactions, or shifts in central hormone regulation.
