Overview and History

FSH is a heterodimeric glycoprotein hormone secreted by the anterior pituitary. It shares a common alpha subunit with other glycoprotein hormones and a unique beta subunit that confers receptor specificity; FSH acts on gonadal cells via the FSH receptor to modulate gametogenesis and local production of regulatory proteins. Clinically, FSH is a core marker of the hypothalamic–pituitary–gonadal axis. Its release is regulated by GnRH from the hypothalamus and feedback from sex steroids and inhibins. Changes in FSH reflect shifts in axis signaling, reproductive-stage transitions, and responsiveness of gonadal tissue rather than a single isolated process.

Interpreting Highs and Lows

Elevated FSH often signals reduced negative feedback from the gonads or lower gonadal responsiveness to pituitary signaling; this may be seen with age-related shifts or when the gonads are less responsive to stimulation. Low FSH tends to reflect reduced pituitary output or increased negative feedback from circulating sex hormones, external hormone exposure, or hypothalamic–pituitary suppression. Interpreting high or low FSH requires the full panel and clinical context—cycle phase, age, concurrent hormones (LH, estradiol, testosterone, inhibin), medications, and health history all influence meaning.
