Overview and History

Iron is a transition metal (Fe) and an essential micronutrient in human biology. It exists mainly in ferrous (Fe2+) and ferric (Fe3+) forms and is a core component of heme proteins (like the oxygen-carrying proteins in red blood cells) and many non-heme enzymes that support energy production, DNA synthesis, and antioxidant defenses. Clinically, iron handling is described by pools: circulating iron bound to transport proteins, storage iron held in ferritin, and iron incorporated into cells and enzymes. Laboratory measurement of serum iron and related markers developed to track these pools and to provide insight into oxygen delivery, cellular metabolism, and overall blood health patterns.

Interpreting Highs and Lows

Higher-than-average serum iron values may reflect increased circulating iron from recent intake, increased release from storage pools, or reduced binding capacity in blood. Elevated iron measurements are interpreted together with ferritin and transferrin saturation to clarify whether circulating iron reflects greater tissue availability or a transient change in transport. Lower-than-average serum iron values suggest less circulating iron available for oxygen transport and iron-dependent enzymes.
