Overview and History

MCH is a standard hematology index reported by automated blood analyzers and expressed in picograms (pg) per cell. It belongs to the red cell indices group alongside MCV and MCHC and is derived from measured hemoglobin and red blood cell count. Biologically, MCH reflects the mass of hemoglobin within individual erythrocytes and is influenced by red cell size, hemoglobin synthesis, and the rate of new cell release. Clinically it has been used for decades as part of routine blood work to differentiate patterns of red cell production and hemoglobin packing when evaluated with iron studies, reticulocyte data, and nutrient markers.

Interpreting Highs and Lows

Lower-than-typical MCH values are associated with smaller or less hemoglobinized red cells and patterns seen with reduced iron availability, increased red blood cell loss, or certain inherited hemoglobin assembly differences. Higher-than-typical MCH values are associated with larger red cells, recent release of immature cells, altered liver-related processing, or reduced vitamin B12/folate availability. Because MCH is one component of red-cell assessment, interpretation requires the full panel context—MCV, hemoglobin concentration, reticulocyte count, ferritin, and related markers—to form a coherent pattern.
