Nucleated red blood cells (nRBCs), often called normoblasts, are immature erythroid precursors that normally reside in the bone marrow until they mature and extrude their nucleus to become reticulocytes and then mature red blood cells. As a laboratory category, this measure is part of hematology and reflects cellular stages in erythropoiesis—the process that makes red blood cells. Historically, counting circulating nRBCs has been used in neonatal care and in clinical labs to flag changes in marrow activity or marked shifts in red cell production. Measuring absolute nRBCs complements other red cell indices by showing whether immature precursors are appearing in peripheral blood, which signals altered marrow release or turnover dynamics.

Interpreting Highs and Lows

Elevated absolute nRBC counts indicate that immature red cell precursors are appearing in peripheral blood more frequently.
