Overview and History

Lp-PLA2 (lipoprotein-associated phospholipase A2) is a serine-dependent enzyme also historically referred to as platelet-activating factor acetylhydrolase. It is produced by immune cells such as macrophages and circulates bound mainly to LDL and, to a lesser degree, HDL particles. Biochemically, Lp-PLA2 hydrolyzes oxidized phospholipids on lipoproteins, generating lysophosphatidylcholine and oxidized free fatty acids. Since the 1990s it has been studied as a marker of lipoprotein-associated inflammatory activity and as a complementary readout to conventional lipid and inflammation measures.

Interpreting Highs and Lows

Higher PLAC activity or concentration may indicate increased lipoprotein-associated phospholipid turnover and greater inflammatory signaling linked to oxidized lipids; it often appears alongside elevated atherogenic lipoprotein patterns or other inflammatory markers. Lower PLAC levels can reflect reduced enzyme production or binding, lower circulating lipoprotein substrate, certain genetic variants, or laboratory variability.
