Overview and History

Potassium is an essential mineral and the predominant intracellular cation in human cells. Biochemically it is a monovalent alkali metal ion that works with sodium and chloride to set membrane potential, drive nerve and muscle excitability, and support cellular enzyme function. Clinically, potassium balance is regulated by cellular pumps (notably the Na+/K+ ATPase), renal excretion, and hormones such as aldosterone. Its measurement has long been used to evaluate fluid and electrolyte homeostasis, kidney function, and conditions that alter cellular shifts or tissue breakdown; the element was first isolated in the early 19th century and has been central to electrolyte physiology ever since.

Interpreting Highs and Lows

Higher-than-normal serum potassium levels may reflect reduced renal excretion, release of potassium from tissues into the blood, shifts related to acid-base disturbances, or effects of certain medications and hormones. Hemolysis during blood draw can also spuriously raise measured values. Lower-than-normal levels can arise from excess loss (for example via urine or the digestive tract), intracellular shifts driven by hormones or alkalosis, or inadequate intake in prolonged situations. Interpreting either direction requires correlation with other lab values (creatinine, sodium, acid-base markers, hormones), clinical context, and medication history to reveal patterns rather than a single cause.
