Progesterone is a C21 steroid hormone in the progestogen family, synthesized from cholesterol through pregnenolone and downstream steroid pathways. It was isolated and characterized in the early 20th century and is studied as a central reproductive and neuroactive steroid. Biologically, progesterone binds intracellular progesterone receptors to alter gene expression in target tissues such as the endometrium, breast, brain, and bone. It also has rapid neuromodulatory effects via metabolites that interact with GABA receptors, contributing to sleep, mood, and thermoregulation. Production sources shift with reproductive state: ovarian corpus luteum in the luteal phase, placenta during pregnancy, and smaller contributions from the adrenal cortex.

Interpreting Highs and Lows

Higher circulating progesterone levels are commonly associated with the luteal phase of the menstrual cycle, pregnancy-related placental production, or recent use of exogenous progestogens. Elevated values can also reflect broader shifts in steroidogenesis when interpreted alongside adrenal and gonadal markers. Lower progesterone levels can be observed outside the luteal window, with anovulatory cycles, during the menopausal transition, or when corpus luteum support is reduced.
