RDW-SD (Red Cell Distribution Width — Standard Deviation) is a hematology index reported as the standard deviation of the red blood cell size distribution. It is part of routine complete blood count panels and quantifies anisocytosis, the degree to which cell sizes differ from each other. Technically, RDW-SD measures the width of the volume histogram at the 20% frequency level and is expressed in femtoliters (fL). Clinically, RDW-SD has been used for decades to add context to red cell indices like mean corpuscular volume (MCV) and reticulocyte counts. Biologically, variation in red cell size arises when newly produced reticulocytes enter circulation, when iron or other nutrients are limited during production, or when older cells are removed unevenly—so RDW-SD serves as a window into recent and ongoing changes in red blood cell dynamics.

Higher RDW-SD values indicate greater variability in red blood cell size. This pattern can be associated with mixed populations of small and large red cells, a recent surge of young cells entering circulation, or processes that disrupt uniform red cell production. Lower RDW-SD values reflect more uniform red cell sizes and may be seen when production and maturation are consistent. Interpretation requires clinical context: RDW-SD is most informative when paired with other blood markers such as MCV, hemoglobin, ferritin, and reticulocyte measures. Alone it shows a pattern; combined with the full panel it helps narrow which biological processes—nutritional supply, marrow activity, or recent blood loss—are contributing to that pattern.
