Reticulocyte Hemoglobin (often reported as Ret‑He or CHr) is a laboratory measurement produced by modern automated hematology analyzers. It quantifies the hemoglobin content of reticulocytes, the youngest circulating red cells, and is typically reported in picograms per cell. The test captures a short-term window of bone marrow activity, roughly the prior few days, because reticulocytes circulate briefly before maturing. Clinically, Reticulocyte Hemoglobin emerged as a useful early indicator of functional iron availability and marrow response. Unlike measures that reflect long-term iron stores, this marker directly samples how much hemoglobin is being incorporated into new cells, making it sensitive to recent changes from iron supply, inflammation, blood loss, or shifts in erythropoietic drive.

Higher Reticulocyte Hemoglobin values can occur when recent iron supply and hemoglobin synthesis are robust relative to reticulocyte size, or when erythropoiesis is stimulated. Interpretation should consider concurrent markers of red blood cell production, iron markers, and clinical context. Technical factors and timing after therapy or blood loss can also influence results. Lower Reticulocyte Hemoglobin values are commonly associated with reduced iron available for new hemoglobin synthesis or with impaired incorporation of iron into developing red cells. Low values may appear with recent blood loss, inflammatory iron sequestration, inadequate dietary or functional iron, or ineffective marrow response. Accurate assessment requires comparison with ferritin, transferrin saturation, complete blood count indices, and clinical history.
