Reticulocytes are immature red blood cells normally present in the circulation for a short time after leaving the bone marrow. Reticulocyte indices include the absolute reticulocyte count, immature reticulocyte fraction (IRF), reticulocyte hemoglobin content (Ret‑He or CHr), and related production indexes. These measures fall under hematology markers that track recent erythropoiesis and red cell maturation. Historically, reticulocyte counting has been used to assess marrow response to bleeding or anemia and to monitor recovery after therapy. Newer indices such as Ret‑He provide information about hemoglobin incorporation into newly formed cells, linking iron availability and marrow activity in near‑real time.

Elevated reticulocyte indices may indicate an increased marrow response and higher production of new red cells. This pattern can be seen when the body is replacing lost or destroyed red cells, during recovery from low counts, or when marrow is stimulated; interpretation depends on concurrent red cell indices, hemoglobin, and hemolysis markers. Low reticulocyte indices may reflect reduced marrow production or impaired hemoglobinization of new cells. Possible associations include limited iron availability, insufficient B12 or folate for red cell formation, chronic inflammatory states that suppress production, or other factors that blunt marrow output. These indices are best interpreted alongside iron studies, complete blood count, and clinical context within the full test panel.
