Reverse T3 is a chemically modified form of the thyroid hormone thyroxine (T4). Biochemically it is produced by inner‑ring deiodination of T4 by deiodinase enzymes and is classified as an inactive isomer of triiodothyronine. Clinically it was characterized as a distinct T4 metabolite when researchers mapped peripheral thyroid hormone metabolism and noted differing biologic activities among isomers. Although rT3 itself has minimal metabolic activity, it is part of the peripheral regulation of thyroid signaling. Changes in the balance between rT3 and active T3 reflect shifts in enzyme activity and whole‑body metabolism, acting as a marker of how the body adapts thyroid output to stress, nutritional state, inflammation, or pharmacologic influences.

Interpreting Highs and Lows

Elevated rT3 levels may reflect increased conversion of T4 into inactive forms relative to active T3. Such patterns are often associated with acute or chronic physiologic stressors, metabolic slowdown, changes in inflammatory tone, fasting or calorie restriction, and certain medications. Low rT3 levels can indicate more efficient conversion of T4 into active T3 or simply reduced T4 substrate; they may also result from laboratory variability.
