Overview and History

SHBG (sex hormone–binding globulin) is a circulating glycoprotein synthesized mainly by the liver. It binds sex steroids with high affinity and circulates largely as a dimer; SHBG measurement has long been used to refine interpretation of total hormone concentrations in clinical testing. Biochemically, SHBG determines the partitioning between protein-bound and free hormone fractions, which affects hormone delivery to target tissues. Its levels respond to hormonal signals, metabolic state, age, and liver synthetic activity, making SHBG a marker of endocrine context rather than an isolated hormone value.

Interpreting Highs and Lows

Elevated SHBG generally corresponds with a higher proportion of hormone bound to protein and a lower free hormone fraction. Higher SHBG is commonly associated with greater estrogen exposure, increased thyroid activity, older age, pregnancy, or other influences that increase hepatic SHBG synthesis. Low SHBG typically corresponds with a higher free hormone fraction and is associated with higher insulin levels, increased adiposity, androgenic influences, and metabolic states that lower hepatic SHBG production.
