Overview and History
Bilirubin is a tetrapyrrole pigment formed when the heme portion of hemoglobin is broken down. Biochemically it exists in two main forms: unconjugated (lipid-soluble) bilirubin produced by macrophages and red blood cell turnover, and conjugated (water-soluble) bilirubin created in the liver by glucuronidation to enable excretion.
Historically, bilirubin measurements became clinically important as a simple blood marker reflecting the balance between production, liver processing, and excretion. Laboratory assays report total bilirubin as the sum of these forms, which helps track organ function and systemic processes that influence heme clearance.
Interpreting Highs and Lows
Elevated total bilirubin can reflect higher production of bilirubin (for example, increased red blood cell breakdown), reduced hepatic conjugation activity, or impaired bile excretion. Laboratory factors and transient physiological states can also raise levels, so patterns across related liver and blood markers help clarify cause.
Low total bilirubin is less common and may reflect reduced production or faster clearance; minor decreases are often of limited clinical concern. Interpretation should always consider hemoglobin, liver enzymes, bile markers, and the full clinical panel to identify meaningful patterns rather than a single-value conclusion.
