Overview and History

Total T4 is a biochemical measurement of thyroxine (T4), an iodine-containing hormone produced by the thyroid gland. Biochemically it is an iodinated tyrosine derivative and circulates mostly bound to carrier proteins (thyroxine-binding globulin, albumin, transthyretin) with a small free fraction that is biologically active. Historically, Total T4 was one of the first widespread thyroid assays and remains clinically useful because it captures both hormone production and the influence of binding proteins. T4 acts as the main circulating thyroid hormone and serves as a reservoir that tissues can convert to the more active form, T3, supporting metabolic and developmental processes.

Interpreting Highs and Lows

Higher Total T4 values may be associated with increased thyroid hormone production, excess circulating hormone from external sources, or increased levels of binding proteins (for example during pregnancy or with estrogen exposure). Lower Total T4 values may reflect reduced hormone production, low binding-protein levels, recent severe illness, malnutrition, or certain medications. Interpretation of high or low Total T4 is most informative when viewed with free T4, TSH, clinical context, and other Foundation Zoomer markers that affect binding, metabolism, and protein status.
