Transferrin is a glycoprotein synthesized primarily by the liver that binds ferric iron (Fe3+) and carries it in plasma. It is a central component of systemic iron handling and is commonly assessed alongside related measurements such as total iron-binding capacity (TIBC) and transferrin saturation. Recognized historically as the main iron carrier in blood, transferrin helps deliver iron to cells while minimizing free iron that can drive oxidative reactions. Its circulating concentration is influenced by iron stores, nutritional status, and inflammatory signals; levels often change when the body’s iron balance or protein synthesis is altered.

Interpreting Highs and Lows

Elevated transferrin often reflects increased iron-binding capacity and is commonly associated with lower iron availability or increased iron demand; certain physiologic states and hormonal influences can also raise transferrin. Low transferrin is frequently linked with reduced liver protein production, systemic inflammation, poor protein nutrition, or protein loss.
