Transferrin Saturation (TSat) is a calculated laboratory index defined as the ratio of serum iron to total iron‑binding capacity, expressed as a percent. Transferrin is a liver-produced glycoprotein that carries iron through the bloodstream to bone marrow and other tissues. TSat has been part of standard iron studies for decades to characterize iron availability in the circulation. Biochemically, it describes the proportion of transferrin binding sites occupied by iron and provides a snapshot of circulating iron exchange rather than total body iron stores.

Interpreting Highs and Lows

Elevated TSat may indicate a greater proportion of transferrin occupied by iron, suggesting higher circulating iron availability; very high values can be associated with increased non-transferrin-bound iron risk. Low TSat indicates a smaller proportion of transferrin carrying iron, suggesting reduced iron availability for tissues. Interpretation always depends on complementary markers — ferritin, TIBC, serum iron, CBC, reticulocyte indices and inflammatory markers — and on the overall pattern seen across the Foundation Zoomer panel.
