Overview and History

Estriol (E3) is a steroid estrogen and one of the primary metabolites formed during estrogen biosynthesis. Biochemically it belongs to the family of estrogens alongside estradiol and estrone; its circulating levels reflect both local tissue metabolism and systemic production. Historically, estriol drew clinical attention because its levels rise markedly in pregnancy and it serves as a window into placental and fetal steroid activity. Biologically, E3 is a weaker agonist at estrogen receptors than estradiol, so its role is more indicative of cumulative estrogen processing and clearance than of strong direct estrogenic signaling.

Interpreting Highs and Lows

Elevated estriol levels often reflect increased estrogen production or metabolic shifts that favor E3 formation and/or reduced clearance; during pregnancy, higher E3 is typical as placental steroid output increases. Elevated nonpregnancy E3 can also associate with greater peripheral conversion of androgens to estrogens or environmental estrogen exposure. Low estriol levels may indicate reduced upstream estrogen synthesis, altered placental-fetal steroid activity in pregnancy contexts, or shifts in metabolic pathways that divert estrogen metabolism away from E3.
