Unfold the stool collection sheet containing 2 flaps and orient the curve notch facing you.

Remove the protective covering from the tape and adhere the stool collection sheet onto the toilet seat.

Note: The stool collection sheet should not touch the surface of the water.

Have a bowel movement.

Note: Make sure stool does not come in contact with the toilet water or urine. Slowly drain urine away from bowel movement before collection a sample if necessary.

Scoop is attached to the cap of the tube. Choose well-formed, diarrheal, and bloody area of stool if present. Collect stool samples in both tubes.

Orange cap tube: Do not dispose fluid. Add stool inside the liquid to raise level of liquid to fill line.

White cap tube: Fill stool all the way up to the line.

Screw the cap tightly.

Remove stool collection sheet flaps from toilet. Flush both the sampling sheet and excess stool down the toilet.

Note: When using water saving toilets, use a full flush. Repeat if necessary.

Place BOTH TUBES into the biohazard bag with absorbent pad and seal completely.

Write Date and Time of Collections on the label affixed to the biohazard bag. Keep the biohazard bag inside the small box provided and place it into the return envelope.

Mail the package within 12 hours of collection via FEDEX. HOLDING COLLECTED SAMPLES CAN RESULT IN REJECTION.

*This step is important as any missing information will delay the processing of your specimen.
