Collect your urine in the urine collection cup provided until the fill line

Unscrew the cap from the specimen tube. Transfer the urine from the cup to the tube until it reaches the 15 ml line.

Screw the required specimen tube securely by twisting tightly. Please discard the cup after use.

Place the specimen tube required into the biohazard bag with the absorbent pad and seal completely.

Write Date and Time of Collection on the label affixed to the biohazard bag.

*This step is important as any missing information will delay the processing of your specimen.

Place the biohazard bag inside the return envelope

Mail immediately via FedEx. Holding collected samples more than 12 hours may result in rejection.
