Overview and History
Glutamic Acid Decarboxylase 67 (GAD67) is one of two major isoforms of the glutamic acid decarboxylase enzyme family that catalyzes the conversion of glutamate into gamma-aminobutyric acid (GABA), a primary inhibitory neurotransmitter in the central nervous system. GAD67 is encoded by the GAD1 gene and is expressed widely in the brain; it contributes to baseline, cell-wide GABA production rather than rapid, synaptic release.
Clinically, GAD67 has been studied as a target of immune reactivity because antibodies can form against GAD proteins. Laboratory detection of immune responses to GAD67 complements measurements to GAD65 and other neural or endocrine markers, helping map where immune activity may intersect with GABAergic neurons or GAD-expressing peripheral cells.
Interpreting Highs and Lows
Elevated immune reactivity to GAD67 (higher autoantibody levels) may indicate immune targeting of tissues that express GAD67, such as GABA-producing neurons and certain endocrine cells. Such a pattern can be associated with neurological or endocrine immune activity, but interpretation depends on symptom pattern and other panel results.
Low or absent immune reactivity to GAD67 suggests a lack of measurable autoantibodies against this isoform at the time of testing. A low result does not rule out immune activity elsewhere; it should be interpreted alongside the full Immune Zoomer panel and clinical context to identify broader patterns.
